Overview

The experience of living and working with schizophrenia is often fraught with challenges and setbacks. This book is a comprehensive attempt to explain why, in spite of near-miraculous advances in medication and treatment, persons with mental illness fare worse than almost any other disadvantaged group in the labor market. As a researcher of economics and disability and the mother of a son with schizophrenia, the author is able to speak from both professional and personal experience and authority. First, she looks at societal factors that affect employment outcomes for persons with schizophrenia (or other serious mental illness), including: stigma and discrimination, investments in human capital, the quality of mental health services, and the support of family and friends. Then she examines workplace factors that affect employment outcomes, including: employer mandates in the Americans with Disabilities Act, the decision to disclose a diagnosis of mental illness at work, the interaction between job demands and functional limitations, and job accommodations for persons with a serious mental illness. Giving weight to both perspectives, the final chapter sets forth a set of policy recommendations designed to improve employment outcomes for this population. Through her research and experience, Marjorie Baldwin concludes that the general public, and many mental health professionals, vastly underestimate the productive capacities of many people with schizophrenia and other serious mental illnesses. She further suggests that a centralized mental health system, applying one-size-fits-all policies to a diverse caseload, is ill-equipped to help individuals with schizophrenia achieve their productive potential. Our best efforts to secure productive work for persons with schizophrenia have not placed the majority of clients in mainstream jobs that yield stable employment and financial security. Negative stereotypes and low expectations create an invisible cage relegating persons afflicted with the disease to menial jobs and government support. Nevertheless, hundreds of thousands of persons living with schizophrenia have found their own way to productive and rewarding employment. They encounter challenges, such as whether or not to disclose their illness to an employer, or how to secure necessary job accommodations. We have learned almost nothing from the success stories of persons living and working with schizophrenia because we do not hear enough about them. Here, one woman tells one success story and how she and her son got there. Finally, through this thorough treatment, readers will see that many more persons with schizophrenia could obtain stable, gainful employment if providers, employers, and caregivers were equipped with the knowledge to help them.
